Use Self-Determination Program Funds for Adaptive Recreation

Wildflower provides individualized bike coaching, community outings, and outdoor recreation for children, teens, and adults. I help families coordinate with their Financial Management Service so getting started feels clear, organized, and manageable.

Services may be funded as Community Integration Supports under Service Code 331 when included in the participant's IPP and spending plan and authorized through their FMS.

Using SDP funding with Wildflower

Each participant's plan is different, but the onboarding process can be straightforward.

Confirm the service in the participant's plan

Check with the participant's regional center, independent facilitator, or planning team to confirm that community integration or adaptive recreation is included in the IPP and spending plan.

Connect Wildflower with the FMS

Share the participant's FMS contact information. Wildflower can complete the provider onboarding steps and supply requested business, insurance, rate, and service documentation.

Complete the participant intake

Tell me about the participant's goals, interests, communication, sensory needs, safety considerations, and schedule so services can be planned around the individual.

Recreation built around meaningful participation

Bike coaching & supported rides

Patient coaching can support balance, coordination, safe riding skills, confidence, independence, and participation in community recreation.

Community outings & city adventures

Personalized outings may include nature, museums, art, food exploration, ferry trips, city quests, and other activities connected to the participant's interests and goals.

Individual and small-group support

Most services are offered one-to-one or with one staff member for every two participants so pacing, communication, and safety support can remain individualized.

San Francisco Bay Area service area

Most sessions take place in San Francisco, including Golden Gate Park and other car-free or low-risk community spaces. Marin County and East Bay requests are considered by arrangement.

Important: Funding is not automatic. The participant's planning team and FMS must confirm that the service is included in the spending plan and that Wildflower has completed any required onboarding before funded services begin.

Wildflower can provide
